Ivan Candelaria ATC/L,CSCS, CPT

Ivan is presently the Director of Sports Rehabilitation at Peak Performance Rehabilitation in Staten Island NY. Ivan is also the Owner/Founder of RAD Radical Athletic Development consulting. He received his professional degrees at the University of Connecticut. At UCONN, he was the assistant trainer of the 1999 NCAA Men’s Basketball National Championship Team. He later went on to complete a competitive internship with the New Jersey Nets. Ivan has worked with a number of our country’s finest professional athletes who currently play in the NFL, NBA, WNBA and MLS.

He is also currently the Director of Sports Performance at Xtreme Fitness in Edison NJ, where he focuses his attention to high school, college athletes and NFL combine training. Ivan has recently enjoyed traveling the country in his role as a rehabilitation consultant for a number of elite Division I Universities. His efforts have developed and enhanced performance in hundreds of athletes nationwide.

Ivan’s success in the treatment of various injuries is based on his unique approach. He is thorough in recognizing dysfunction in movement which he then corrects with pain-free corrective exercise. Ivan focuses on the elimination of dysfunction that is the root of pain in addition to treating the painful symptoms. His best work is rehabilitation of the post surgical patient including treatment of the “itis’s”, or chronic inflammations. Over 14 years, Ivan has offered professional rehabilitation in addition to athletic training services.

Ivan uses tools such as kenisiotaping, functional movement screening, manual therapy, nerve flossing and corrective exercise techniques to eliminate pain and enhance performance. Ivan is also called upon yearly by NFL agents to perform client visits at Athletes Performance in Carson CA, and IMG Academies in Bradenton FL, to assist his college football players who are training for the NFL Combine. Ivan mentors NCAA college football player entering the NFL draft with the process of selecting a training facility, agent, and financial advisor. He is currently consulting and rehabbing injured high school athletes at Tottenville H.S. in Staten Island, NY., St Joseph’s by the Sea H.S. Staten Island, NY, and Pope John Sparta NJ. Ivan also enjoys mentoring high school athletes through the recruiting process at these schools.

Ivan has also been instrumental in preparing Rutgers finest football players to train for the NFL combine and pro day. He has also lectured to the Rutgers University Medical staff on the practice of the Functional Movement Screen (FMS). A major highlight to his career has been the ability to work with the most respected people in his field such as Dr. James Andrews, Todd Durkin, Travelle Gains, and Grey Cook.


Diana Lee, DPT

  • Undergraduate: Hunter College, BS, Biology, 2009
  • Graduate: Hunter College, Doctorate of Physical Therapy, 2012

Diana is a New York State Licensed Physical Therapist. She attended City University of New York, Hunter College and graduated with honors, receiving her Bachelor of Arts in Biological Sciences and Biochemistry. She later completed her Doctorate of Physical Therapy in Hunter college. During her clinical affiliations in the program, she was exposed to a variety of settings, including acute bedside, subacute care, and outpatient orthopedics. She is currently working as staff physical therapist with Peak Performance/Regional Orthopedics and pursuing a lifelong learning career of becoming a skilled physical therapist.


Sara Ansari, DPT, FMSC

  • Undergraduate: Seton Hall University, BS Major in Biology, Minor in Chemistry
  • Graduate: Northeastern University, Doctor of Physical Therapy, 2012

Sara is a Doctor of Physical Therapy licensed in both New York and New Jersey. While studying at Northeastern, she attended a six month co-op program at Massachusetts General Hospital’s Wang Ambulatory Outpatient Physical Therapy Department. There, she gained experience working on patients with amputations, cardiopulmonary disease, neurological disorders and orthopedic conditions.

She also participated in two geriatric research studies which focused on incorporating WiFit Technology and the Berg Balance Master to minimize fall risk and improve balance and proprioception in the elderly population. Her clinical internships in her last year of the DPT program further developed her clinician skills working on patients suffering from postural deformities, athletic injuries, brain trauma injuries and stroke to geriatric and musculoskeletal disorders.

Sara is also currently certified in Functional Movement Screening (FMS), which allows her to use a 7-step exercise screen to improve her patients’ movement patterns. She is currently a fulltime staff physical therapist at Regional Orthopedics, where she hopes to use her skills in treating each patient with a whole body approach to fully rehabilitate them back to their prior level of function.


Dustin Garlock PT, DPT

  • Graduate: Ithaca College, Doctor of Physical Therapy, 2013

Dustin attended Ithaca College where he majored in Clinical Health Studies and minored in health. During his undergraduate years, Dustin worked at a Skilled Nursing Facility and worked summers as a Physical Therapy Aide. Dustin continued his education at Ithaca, where he graduated from the physical therapy program as a Doctor of Physical Therapy.

During his clinical affiliations Dustin’s work included acute and subacute settings, stroke/spinal cord rehabilitation in addition to outpatient orthopedic/sports physical therapy. Dustin is currently working as a physical therapist at Peak Performance/Regional Orthopedics in his pursuit of becoming a highly trained physical therapist.


Jessica Lalicata, PTA

  • Undergraduate: Touro College, AAS, Physical Therapist Assistant, 2001

Jessica was born in Brooklyn, NY and raised in Staten Island, NY. She is licensed in both New York and New Jersey. As class representative at Touro College, having earned an American Physical Therapy Association Leadership Award, Jessica graduated with honors. Throughout her 13+ years of experience, Jessica has worked in a variety of settings including: acute, inpatient and outpatient hospitals; sub-acute care facilities; and outpatient orthopedic and sports medicine facilities.

Jessica specializes in the treatment of vestibular disorders and orthopedic injuries and she is also a certified Nerve Conduction Velocity Technician. Jessica is a caring, empathetic professional with only the best interests of the patient in heart and mind. Her main focus when treating patients is manual therapy and therapeutic exercise with the goal of restoring patients to normal functional mobility. Jessica is currently working as a staff physical therapist assistant with Peak Performance/Regional Orthopedics.


Christine Chimento

Christine Chimento graduated from Quinnipiac University with a Masters of Physical Therapy Degree and from Temple University with a Doctorate of Physical Therapy Degree. She has been practicing in outpatient orthopedics since 2005 and is licensed in the states of NY and NJ. Christine combines her positive outlook and attitude with true compassion to help patients return to work and sports without pain. She is a true believer in continued education in order to learn the newest techniques and medical procedures to better herself and her patients.
